This tutorial was written by me on July 31, 2013.
It was made out of my own creation and any similarity is purely coincidental.
I used PSPx2 but any version would work.

Here goes:

Let’s start with the mask layer:
New Raster – 800 X 800.
Open Paper 11.  Open mask and minimise.
SELECTIONS – SELECT ALL – Copy and paste Paper 11 into selection.  
 Go to LAYERS –
NEW MASK LAYER – FROM IMAGE – SOURCE LUMINANCE - CHECKED – from menu, find WSL Mask 84 – OK.  Re-size 110%.

Let’s do the frame layer now:
Element 111 – C/P and move just a little bit down.
Grab your magic wand.  Click in the middle of frames. 
SELECTIONS – MODIFY – EXPAND 4 PIXELS. 
New Raster Layer.  Move under frame layer.  Open Paper 12.
Copy and paste into selection.
Grab selected CU of tube.  C/P and move into position.  Re-size 90%. SELECTIONS - INVERT - DELETE - INVERT.
Duplicate tube layer.
ADJUST – BLUR – GAUSSIAN BLUR – RADIUS 25 - OK
Change BLEND MODE of copy of tube layer to HARD LIGHT and go to original tube layer and change BLEND MODE to LUMINANCE (L) and reduce opacity to 65.  (see tag for placements)

Now for some elements:
Element 64 – C/P and re-size 85%.  Move to to of tag over mask layer but under frame layer.

Element 39 – C/P and leave where it lands over frame layer.  It will be part of the next two elements.

Element 78 – C/P and re-size 25%.  Leave over element 39 for now.  Click inside of this layer.  SELECTIONS - MODIFY - EXPAND 4 PIXELS.  New raster layer.  Move under element 78 layer.

Element 53 – Copy and paste into selection.  Highlight element 39 layer.  Merge down and then merge down again so that element 39, 78 and 53 are now one layer.  Now move this to the upper left corner of tag.
(see tag for placement) Element 50 – C/P and re-size 90%.  Move to bottom left corner of tag.

Element 13 – C/P and re-size 75%.  Move in front of element 50.
(see tag for placement)
Element 22 – C/P and re-size 30%.  Move to top of element 50.
(see tag for placement)
Element 69 – C/P and re-size 40%.  Move to bottom left of tag over element 22.
(see tag for placement)
Element 11 – C/P and re-size 40%.  Move to bottom left corner of tag in front of element 50 but left side of element 22.
(see tag for placement)  Element  68 - C/P and re-size 40%.  Move to top right side of tag. (see tag for placement)
Element  92 - C/P and re-size 90%.  Move to bottom of tag in front of element 13
(see tag for placement)
Element 99 -  C/P and re-size 90%.  Move to bottom right side of tag over bottom flower on frame.

Element 9 - C/P and re-size 30%.  Move over top of element 13 and to the left.
(see tag for placement)
Now the tube layer:
If you are using the same tube as me,
C/P tube and re-size 90%.  Move over to right side of tag.

Add drop shadow to layers of your choosing.
I used
V & H 2
Opacity 70
Blur 7
Color Black

 Now you can add name and  © information.  Save as .png and re-size if you wish.
I re-size before I save and add name and © info.
