This tutorial was written by me on November 7, 2013.
It was made out of my own creation and any similarity is purely coincidental.
I used PSPx2 but any version will do.

Here goes:

Let’s start with the mask layer:
New Raster – 800 X 800.
Open Paper 7.  Open mask and minimize.
SELECTIONS – SELECT ALL – Copy and paste Paper 7 into selection.
Go to LAYERS – NEW MASK LAYER – FROM IMAGE – SOURCE LUMINANCE - CHECKED – from menu, find WSL 84 – OK.  Duplicate and merge down.  Re-size 115%.

Now for the frame:
Element 28 - C/P and move up and a to the left.  Make sure your frame layer is highlighted.  Grab your magic wand.  Click in the middle of the frame.   
Go to SELECTIONS - MODIFY - EXPAND 8 PIXELS - INVERT.   
C/Paste into selection Paper 10.  Keep selected.  
Now grab the tube you are using.  If you are using the same one as me, get it now.  C/P and re-size 40%.  Move her into position.  Do this three more times.  Mirror one of the copies.
SELECTIONS - INVERT - DELETE - SELECTIONS - INVERT.  Merge all layers down.  Duplicate the tube layer.
ADJUST – BLUR – GAUSSIAN BLUR – RADIUS 25 - OK.
Change BLEND MODE of copy of tube layer to HARD LIGHT and change the original layer to OVERLAY.

Lets do some more elements now:
Element 12 - C/P and move this layer under frame layers and move to top left corner of tag.  Duplicate and mirror.

Element 21 - C/P and move to right side of tag.
(see tag for placement)
Element 22 - C/P and move to top left corner over frame.
(see tag for placement)
Element 3 - C/P and move to bottom right corner of tag.
(see tag for placement)
Element 2 - C/P and move to bottom left corner of tag.
(see tag for placement)
Element 26 - C/P and free rotate 90º right.  Move to bottom right side of tag in front of element 3.  Duplicate and mirror.
(see tag for placement)

Now for the tube(s):
If you are using the same tube as me, C/P and re-size 30%.  Move to bottom middle of tag.

Add drop shadow to layers of your choosing.
I used: 
V & H 2
Opacity 70
Blur 7
Color Black

 Now you can add name and  © information.  Save as .png and re-size if you wish.

I re-size before I save and add name and © info.

Here goes:

Let’s start with the mask layer: 
New Raster – 800 X 800.
Open Paper 3.  Open mask and minimize.
SELECTIONS – SELECT ALL – Copy and paste Paper 3 into selection.
Go to LAYERS – NEW MASK LAYER – FROM IMAGE – SOURCE LUMINANCE - CHECKED – from menu, find WSL 84 – OK.  Duplicate and merge down.  Re-size 120%.

Let’s do the frame layer now:

Element 10 - C/P and move up and a little left on tag.
Grab your magic wand.  Click in the middle of frame.
SELECTIONS – MODIFY – EXPAND 8 PIXELS.  New Raster Layer.  Move under frame layer.  Open Paper 2.  Copy and paste into selection.  Keep selected.  If you are using the same tube as I am, grab it, C/P and mirror then move it into place. 
SELECTIONS - INVERT - DELETE - SELECTIONS - INVERT.
Duplicate the tube layer.  ADJUST – BLUR – GAUSSIAN BLUR – RADIUS 25 - OK
Change BLEND MODE of copy of tube layer to HARD LIGHT and go to original tube layer and change BLEND MODE to SCREEN

Now for some more elements after tube layers:
Element 25 - C/P and move up on tag just a little.
(see tag for placement)
Element 26 - C/P and re-size 70%.  Move to bottom right side of tag.
(see tag for placement)
Element 27 – C/P and re-size 70%.  Move to middle left side of tag beside frame layer.
(see tag for placement)
Element 6 - C/P and re-size 50%. Move to left side in front of element 27.
(see tag for placement)
Element 28 - C/P and re-size 50%.  Move to lower left side in front of element 6.
(see tag for placement)
Element 14 - C/P and re-size 50%.  Move to bottom left to the right of element 28.
(see tag for placement)
Element 5 - C/P and re-size 50%.  Move to bottom left, but to right side of element 14.
(see tag for placement)
Element 15 - C/P and re-size 50%.  Move to bottom left and to the left of element 5.
(see tag for placement)
Element 16 - C/P and re-size 50%.  Move to bottom left but to left side of element 15.
(see tag for placement)   

Now for the tube(s):
If you are using the same tube as me, C/P the full size of tube and re-size 50%.  Move to bottom right of tag in front of element 26.

Add drop shadow to layers of your choosing.

Let’s start with the mask layer: 
New Raster – 800 X 800.
Open Paper 12.  Open mask and minimize.
SELECTIONS – SELECT ALL – Copy and paste Paper 12 into selection.
Go to LAYERS – NEW MASK LAYER – FROM IMAGE – SOURCE LUMINANCE - CHECKED – from menu, find WSL 84 – OK.  Duplicate and merge down.  Re-size 110%.

Let’s do the frame layer now:

C/P and move it into position.
Go to SELECTIONS - INVERT - DELETE - SELECTIONS - INVERT.
  Duplicate the tube layer.  ADJUST – BLUR – GAUSSIAN BLUR – RADIUS 25 - OK
Change BLEND MODE of copy of tube layer to HARD LIGHT and go to original tube layer and change BLEND MODE to SCREEN.

Element 6 - C/P and re-size 30%.  Move up over paper but under frame.

Element 3 - C/P and re-size 80%.  Move up on paper layer.
(see tag for placements)  
Element 4 - C/P and re-size 50%.  Move left on paper layer.

Element 5 - C/P and re-size 50%.  Move left on paper layer but in font of element 4.

Gonna do a few elements after frame layer:
Element 13 - C/P and move to the top of the frame.
(see tag for placement)
Element 12 - C/P and re-size 70%.  Move to left side of frame.
(see tag for placement)
Element 17 - C/P and move to bottom left side of frame.
(see tag for placement)
Element 8 - C/P and re-size 55%.  Move to bottom left side of tag.
(see tag for placement)
Element 10 - C/P and re-size 30%.  Move to bottom left over element 8.
(see tag for placement)
Element 2 – C/P and re-size 45%.  Move to bottom left side of tag but right of element 8.
(see tag for placement)
Element 23 – C/P and re-size 30%.  Move to bottom left corner of tag.

Element 20 – C/P and re-size 30%.  Move to bottom left but to the right of element 23.
(see tag for placement)
Element 1 – C/P and re-size 30%.  Move to bottom left but to the right of element 20.

Element 27 - C/P and re-size 30%. Move to bottom left side but to the right of element 1.

Here goes:

Let’s start with the mask layer: 
New Raster – 800 X 800.
Open Paper 1.  Open mask and minimize.
SELECTIONS – SELECT ALL – Copy and paste Paper 1 into selection.
Go to LAYERS – NEW MASK LAYER – FROM IMAGE – SOURCE LUMINANCE - CHECKED – from menu, find WSL 84 – OK.  Duplicate and merge down.  Re-size 120%.


Let’s do the frame layer now: 
Element 6 – C/P and leave in the middle of the tag.  From here, I grabbed my SELECTION TOOL and created a circle.

Then I added a new raster layer.  Opened Paper 15.  Copied and pasted into selection.  Then I grabbed the magic wand again and went to the frame layer.  Click in the middle of element 6.
SELECTIONS – MODIFY – EXPAND 8 PIXELS - DELETE and was left with a background for the frame layer.  Keep selected.
New Raster Layer.  Move under frame layers.  Open Paper 18.
Copy and paste into selection.  Keep selected.
If you are using the same tube as me, grab the version you like.
C/P and move it into position.  
SELECTIONS - INVERT - DELETE - SELECTIONS - INVERT.  Duplicate the tube layer.  ADJUST – BLUR – GAUSSIAN BLUR – RADIUS 25 - OK
Change BLEND MODE of copy of tube layer to HARD LIGHT and go to original tube layer and change BLEND MODE to LUMINANCE (L).

Now for some elements:

Element 22 – C/P and move to top left corner.  Move this layer under all other layers.  Duplicate and mirror.

Element 13 – C/P and re-size 40%. Move to bottom left side in front of all layers.
(see tag for placement)
Element 34 – C/P and re-size 30%.  Move bottom right side in front of element 13
(see tag for placement)
Element 30 – C/P and re-size 50%.  Move to bottom right side of element 34.
(see tag for placement)
Element 29 – C/P and re-size 50%.  Move to bottom right corner of tag.  Duplicate and move right just a little.
(see tag for placement)
Element 11 – C/P and re-size 25%.  Move to bottom right side of tag.
(see tag for placement)
Element 10 – C/P and re-size 40%.  Move to bottom right side of tag.
(see tag for placement)
Element 4 - C/P and re-size 20%.  Move to bottom right side of tag.
(see tag for placement)
Element 17 - C/P and re-size 50%.  Move to right side of frame.  Duplicate and move up some.
(see tag for placement)
Element 21 - C/P and re-size 50%.  Move to right side between element 17 and copy of element 17.  Make sure copy of element 17 is under this layer.

Element 5 - C/P and re-size 80%.  Move to top left corner of tag.

Element 26 - C/P and move to upper left side of tag.  Duplicate and mirror and flip.
