Here goes: 

The mask layer.

Open Paper5 - Hold down SHIFT key and press the "D" key to duplicate paper. 

Close PP012.

Add NEW RASTER LAYER and go to LAYERS -  ARRANGE - SEND TO BOTTOM. 

Highlight paper layer.

 LAYERS - NEW MASK LAYER - FROM IMAGE - SOURCE LUMINANCE CHECKED - Mask By Lise 5 - OK - MERGE GROUP.  

Duplicate and merge down.

Re-size to  700 X 700.

Now for the elements.

 Element 22- C/P and move a little bit to the left of tag.

Element 109 - C/P and move to of tag.

(see tag for placement)

Element 94- C/P and move to left of tag.  Duplicate and mirror.

(see tag for placement)

Element 38 - C/P and move to right of tag.  Duplicate and mirror.

(see tag for placement)

Element 108 - C/P and move to top middle of tag.

(see tag for placement)

Element 119 -  C/P and move to bottom left of tag.  Duplicate and mirror.

(see tag for placement)

Element 10 - C/P and re-size 70%.  Move to bottom left of tag.  Duplicate and mirror.

(see tag for placement).

Element 30 - C/P and re-size 40%.  Move to right of tag.  Duplicate and mirror.

(see tag for placement)

Element 57 - C/P and re-size 50%.  Move to bottom middle of tag.

(see tag for placement)

Element 63 - C/P and re-size 40%.  Move up and on the right of tag.

SELECTIONS - SELECT ALL - FLOAT - DEFLOAT

Open tube, C/P and move over Element 63 (see tag for placement).

SELECTION - INVERT - DELETE.  Keep selected.

Duplicate tube layer.  Highlight original tube layer - ADJUST - BLUR - GAUSSIAN BLUR - RADIUS 25.00.  Change blend mode to Luminance (L)

Go to Copy tube layer.  Change blend mode to screen and lower opacity to 59

Element 104 - C/P and re-size 40%.  Move to bottom left of tag.

Element 51 - C/P and re-size 30%.  Move over Element 104.

(see tag for placement)

Element 48 - C/P and re-size 30%.  Move up and over to the left of tag.

(see tag for placement)

Now for the Tube Layer.

Open tube again.  C/P and re-size 60%. Move over Element 108 layer but lower on the tag.

Color Black

 Now you can add name and  © information.  Save as .png and re-size if you wish.

I re-size before I save and add name and © info.
