This tutorial was written by me on June 17, 2013.
It was made out of my own creation and any similarity is purely coincidental.
I used PSPx2 but any version will do.

Template: Millie's PSP Madness -  Template-72
Mask: Vix Big Mask 018  Mask 5
Eye Candy 4000 - Gradient Glow
Font: Collegiate Black FLF HERE

Here goes: 
 
Open template - Shift D to duplicate and then delete CR layer.
I don't like a white background unless I really need one, so I clear the white background by highlighting the BACKGROUND layer and go to EDIT - CLEAR.
Also, I like working with a bigger work area.  Go to IMAGE - CANVAS SIZE and change the height and width both to 800.

Now onto the layers. 
Highlight layer 1.  SELECTIONS - SELECT ALL - FLOAT - DEFLOAT
Open Paper 9 - C/P - SELECTIONS - INVERT - DELETE - SELECTIONS - SELECT NONE
Delete layer 1.
Highlight layer 2.  SELECTIONS - SELECT ALL - FLOAT - DEFLOAT
Open Paper 3 - C/P - SELECTIONS - INVERT - DELETE - SELECTIONS - SELECT NONE
Delete layer 2.
Highlight layer 3. SELECTIONS - SELECT ALL - FLOAT - DEFLOAT
Open Paper 8 - C/P - SELECTIONS - INVERT - DELETE - SELECTIONS - SELECT NONE
Delete layer 3. 
Highlight layer 4. SELECTIONS - SELECT ALL - FLOAT - DEFLOAT
Open Paper 3 - C/P - SELECTIONS - INVERT - DELETE - SELECTIONS - SELECT NONE
Delete layer 4.
Highlight layer 7.  SELECTIONS - SELECT ALL - FLOAT - DEFLOAT
Open tube, choose layers, C/P and re-size 80%.  Move into place (see tag for placement).  SELECTIONS - INVERT - DELETE - KEEP SELECTED
Duplicate tube.  On Copy layer, ADJUST - BLUR - GAUSSIAN BLUR - 25.00 RADIUS.  Change blend mode to HARD LIGHT.
Go to original tube layer and change blend mode to EXCLUSION.
Repeat for Layer 8 except for the blend mode on the original layer should be changed to SCREEN.  SELECTIONS - SELECT NONE.

Now for the elements.
Pom 2 - C/P and re-size 50%.  Move to top right of tag.

Pom 1 - C/P and re-size 50%.  Move to top right of tag under Pom 2.
(see tag for placement)
  Pennant - C/P and re-size 50%.  Move to top right of tag between Pom 1 & 2.
(see tag for placement)
Boom Box - C/P and re-size 30%.  Move a bit to right side bottom of tag.
(see tag for placement)
Mega Phone - C/P and re-size 50%.  Move to right side of tag.
(see tag for placement)
Shoe - C/P and re-size 20%.  Move right side of tag.
(see tag for placement)
Word Art 1 - C/P and move to middle right of tag.
I used Eye Candy 4000 on this.  See images below for settings:


 
Now for the tube.
 Open tube.  Choose layers you like, C/P and re-size 50%.  
Move to left side of tag.
(see tag for placement) 

Now, the mask layer.
Highlight very bottom layer.
Copy Paper 5
SELECTIONS - SELECT ALL - NEW RASTER LAYER - PASTE INTO SELECTION - SELECT NONE
LAYERS - NEW MASK LAYER - FROM IMAGE - SOURCE LUMINANCE CHECKED - VBM018 - MERGE GROUP
Move upper right of tag.  DUPLICATE MASK - MIRROR and move down and left of tag.

Add drop shadow to all layers of your choice.  I use:
I used
V & H 2
Opacity 70
Blur 7
Color Black

 Now you can add name and  © information.  Save as .PNG and resize if you wish.
I resize before I save and add name and © info.
