This tutorial was written by me on August 22, 2013.
It was made out of my own creation and any similarity is purely coincidental.
I used PSPx2 but any version would work.

Here goes:
The Template first:
Open template in PSP.  Hold down SHIFT button and tap your “D” key.  Close the original template.  Delete the “Credits” layer with her CR info.
I don't like a white background unless I really need one, so I clear the white background by highlighting the BACKGROUND layer and go to 
EDIT - CLEAR.
Next I changed the canvas size to 800 X 800.  We are gonna do basically the same thing for the following layers, so in an effort to stop repeating myself, here is what you will do for each layer that we change out:

Highlight layer.  SELECTIONS - SELECT ALL - FLOAT - DE FLOAT
Open Paper - C/P - SELECTIONS - INVERT - DELETE - SELECTIONS - SELECT NONE
Delete layer. 
 
So it will go something like this:
You will be changing these layers: Blue Circle 3, Blue Circle 2, Pink Circle 2, Pink Circle 1, Blue Circle 1, Frame Back 3, Frame Back 2, Frame Back 1, Kandi Word Art.  The rest we will leave as is.

Highlight Blue Circle 3.  Follow directions above.  Use PP10.  Rename PP10 left. Flip this layer and move back down where it was.
Highlight Blue Circle 2.  Same as above.  Use PP10.  Rename PP10 right.

Highlight Pink Circle 2.  Same as above.  Use PP8.  Rename PP8 right.
Highlight Pink Circle 1.  Same as above.  Use PP8.  Rename PP8 left.
Highlight Blue Circle 1.  Same as above.  Use PP2.  Rename PP2 middle.
Highlight Frame Back 3.  Same as above.  Use PP1.  Rename PP1 left
Highlight Frame Back 2.  Same as above. Use PP3.  Rename PP3 right.
Highlight Frame Back 1.  Same as above.  Use PP5.  Rename PP5 middle.  
 For Frame Back 3, I grabbed the tube and C/P and re-sized 70%.  SELECTIONS - INVERT - DELETE - INVERT.  Duplicate tube layer. ADJUST – BLUR – GAUSSIAN BLUR – RADIUS 25 - OK Change BLEND MODE of copy of tube layer to HARD LIGHT and go to original tube layer and change BLEND MODE to SCREEN.
For Frame Back 2, I did the same except I mirrored the tube.
For Frame Back 1, I pasted the tube again, but re-sized it 40% and moved her into position.  Then I duplicated it.  Move the copy layer under the frame 1 layer.  Grabbed the magic wand and went to the PP5 middle layer and clicked on the outside of that square.  Went back to the copy of tube layer and hit delete.  SELECTIONS - SELECT NONE.  Grab the magic wand again.  Go to the frame 1 layer, click inside of frame and then go to SELECTIONS - INVERT.  Grab your eraser tool and erase the parts of the tube hanging over the bottom of the frame.
Highlight Kandi WordartSame as above.  Use PP9.  Rename PP9 wordart.  Keep selected.  Go to SELECTIONS - MODIFY - SELECT SELECTION BORDERS and use the following settings.  


And New Raster Layer.  Move under PP9 wordart layer.  Flood fill with white (#ffffff).

Now for some elements:
Element 13 – C/P and re-size 60%.  Move to top middle of tag.  Move this layer under PP1 left.

Element 12 – C/P and move to top right corner of tag.  Duplicate and mirror.

Element 7 – C/P and re-size 40%.  Move to bottom left corner.
(see tag for placement)
Element 8 - C/P and re-size 60%.  Mirror and move to bottom left corner in front of element 7.

Element 17 - C/P and re-size 50%.  Move to bottom right corner.
(see tag for placement)
Element 16 - C/P and re-size 50%.  Move to bottom right corner on left side of element 17.  Free rotate 28º right.

Element 14 - C/P and re-size 60%.  Move to bottom middle of tag.

Element 2 - C/P and re-size 30%.  Move to top right corner over element 12.
(see tag for placement) 

Now for the mask layer:
Open PP4.
Highlight very bottom layer.  SELECTIONS - SELECT ALL
Copy and paste PP4 into Selection.
Go to LAYERS – NEW MASK LAYER – FROM IMAGE – SOURCE LUMINANCE CHECKED – CHOOSE WSL 84 from the drop down menu – OK.  Duplicate and merge down.

Add drop shadow to layers of your choosing.

V & H 2
Opacity 70
Blur 7
Color Black

 Now you can add name and  © information.  Save as .png and re-size if you wish.
I re-size before I save and add name and © info.
