This tutorial was written by me on October 6, 2013.
It was made out of my own creation and any similarity is purely coincidental.
I used PSPx2 but any version will do.

Here goes:

Let’s start with the mask layer: 
New Raster – 800 X 800.
Open PP14.  Open mask and minimize.
SELECTIONS – SELECT ALL – Copy and paste PP14 into selection.
Go to LAYERS – NEW MASK LAYER – FROM IMAGE – SOURCE LUMINANCE - CHECKED – from menu, find WSL 116 – OK.  Duplicate and merge down.

  Gonna do a few elements right now:

Element 120 - C/P and move to far left of tag.

Element 122 - C/P and move to far right of tag.
(see tag for placement)
Element 95 - C/P and re-size 70%.  Move to left in front of element 120.  Duplicate and mirror.  Duplicate copy and move a bit to the left.  Duplicate again and mirror.

Element 93 - C/P and move to bottom left side of tag. Duplicate and mirror.

Element 9 - C/P and move to bottom right in front of copy of element 93.  Duplicate and mirror.

 Element 116 - C/P and re-size 50%.  Move to bottom right side in front of element 97.  Duplicate and mirror.

Element 91 - C/P and re-size 80%.  Move to bottom middle of tag.
(see tag for placement)  

Let’s do the frame layer now: Element 64 - C/P and move to upper left of tag.  Free rotate frame 22º left.  Grab your magic wand.  Click in the middle of frame(s).
SELECTIONS – MODIFY – EXPAND 8 PIXELS.  New Raster Layer.  Move under frame layer.  Open PP19.  Copy and paste into selection.  Keep selected.  If you are using the same tube as I am, grab it and   Re-size 50%.  Free rotate tube 22º left.  Move it into middle of middle frame.
SELECTIONS - INVERT - DELETE - SELECTIONS - INVERT.
Duplicate the tube layer.  ADJUST – BLUR – GAUSSIAN BLUR – RADIUS 25 - OK
Change BLEND MODE of copy of tube layer to HARD LIGHT and go to original tube layer and change BLEND MODE to MULTIPLY and change OPACITY to 46%.

Then open Element 45.  C/P and re-size 80%.  Move into middle of right frame.  Grab magic wand again and go back to frame layer.  Click in the middle of right frame.  SELECTIONS - MODIFY - EXPAND 8 PIXELS - SELECTIONS - INVERT - DELETE - SELECTIONS NONE.  Do the same for the left side of frame except mirror element 45 and then flip. Do SELECTIONS as above.

Now for some more elements after tube layers: Element 92 - C/P and move to far right of tag.  Duplicate and mirror.

Element 106 - C/P and re-size 40%.  Move to bottom over element 91.

Element 99 – C/P and re-size 30%.  Move to top of frame in between right and middle frames.
(see tag for placement)

Now for the tube(s):
If you are using the same tube as me, C/P the tube and re-size 45%.  Move to bottom middle of tag but a little right.

  Add drop shadow to layers of your choosing.

I used: 
V & H 2
Opacity 70
Blur 7
Color Black

 Now you can add name and  © information.  Save as .png and re-size if you wish.
I re-size before I save and add name and © info.
