This tutorial was written by me on October 11, 2013.
It was made out of my own creation and any similarity is purely coincidental.
I used PSPx2 but any version will do.

Here goes:
New Raster – 800 X 800.
Open PP5. Open mask and minimize.
SELECTIONS – SELECT ALL – Copy and paste PP5 into selection.
Go to LAYERS – NEW MASK LAYER – FROM IMAGE – SOURCE LUMINANCE - CHECKED – from menu, find WSL 116 – OK. Duplicate and merge down. Re-size 105%.
Let’s do the frame layer now:
Element 24 - C/P and move to the top part of tag.
Grab your magic wand. Click in the middle of frame.
SELECTIONS – MODIFY – EXPAND 8 PIXELS. New Raster Layer. Move under frame layer. Open PP15. Copy and paste into selection. Keep selected. If you are using the same tube as I am, grab it, C/P and move it into place.
SELECTIONS - INVERT - DELETE - SELECTIONS - INVERT.
Duplicate the tube layer. ADJUST – BLUR – GAUSSIAN BLUR – RADIUS 25 - OK
Change BLEND MODE of copy of tube layer to HARD LIGHT and go to original tube layer and change BLEND MODE to SCREEN and change OPACITY to 67%.

Now for some more elements after tube layers:
Element 83 - C/P and move over PP5 layer. Mirror and move to bottom left side of tag. Duplicate and mirror.

Element 2 - C/P and re-size 60%. Move to middle right side over PP5 layer. Duplicate and mirror.

Element 40 – C/P and move over PP5 layer. Leave in the middle over PP5 layer.

Element 29 - C/P and move to very top of tag over frame layer.

Element 98 - C/P and mirror. Move to bottom of frame layer. Duplicate and mirror and flip.

Element 94 - C/P and move right and to the top of frame layer.

Element 16 - C/P and re-size 85%. Move to bottom of frame layer. Duplicate and mirror and flip.

Element 14 - C/P and re-size 50%. Move to bottom left and to the left of element 5.

Element 43 - C/P and re-size 50%. Move to bottom and on the left side of element 24.

Element 5 - C/P and move to bottom right side in front of element 43.

Element 106 - C/P and re-size 70%. Move to bottom left side of tag in front of element 14.

Element 88 - C/P and move to top right corner of tag. Duplicate, mirror and move down a bit on tag. Duplicate and flip and move down and a little to right.

Add drop shadow to layers of your choosing.
I used:
V & H 2
Opacity 70
Blur 7
Color Black
Now you can add name and © information. Save as .png and re-size if you wish.
