This tutorial was written by me on September 23, 2013.
It was made out of my own creation and any similarity is purely coincidental.
I used PSPx2 but any version will do.

Here goes:

Let’s start with the mask layer: 
New Raster – 800 X 800.
Open PP17.  Open mask and minimize.
SELECTIONS – SELECT ALL – Copy and paste PP17 into selection.
Go to LAYERS – NEW MASK LAYER – FROM IMAGE – SOURCE LUMINANCE - CHECKED – from menu, find WSL 84 – OK.  Duplicate and merge down.  Re-size 110%.

Gonna do a few elements right now:
Element 39 - C/P and move to upper right side of tag.  Duplicate and mirror.

Let’s do the frame layer now: 
Element 35 – C/P and leave it where it lands
Grab your magic wand.  Click in the middle of element. SELECTIONS – MODIFY – EXPAND 8 PIXELS. New Raster Layer.  Move under frame layer.  Open PP11. Copy and paste into selection.

Now for some more elements after frame layers:

Element 32 – C/P and re-size 80%.  Move to right side of tag.  Duplicate and mirror.

Element 59– C/P and re-size 30%.  Move to right side over element 32.

Element 58 – C/P and re-size 30%.  Move to left side over copy of element 32.

I place the tube layer here: 
If you are using the same tube as me, grab it now.  C/P and move into middle of frame.  Make sure it is over element(s) 32, 59, 58.  Duplicate your tube and move the copy under element 35.  Grab your magic wand.  Click in the middle of frame again.  SELECTIONS – MODIFY – EXPAND 8 PIXELS - SELECTIONS - INVERT - DELETE - SELECTION NONE.  Go back to frame layer.  Grab your magic wand and click in the middle of frame again.  SELECTION INVERT and go up to your original frame layer and snag your eraser tool.  Erase the bottom part of tube that is hanging over frame.  SELECTIONS - SELECT NONE.

Element 80 – C/P and move to lower right side over tube layer.

Element 20 - C/P and re-size 60%.  Move to bottom and bit to right side of element 35.

Element 33 - C/P and re-size 75%.  Move to bottom middle of frame.
(see tag for placement) 
Element 76 – C/P and move to bottom right side of tag.  Duplicate and mirror.
(see tag for placement)
Element 19 - C/P and re-size 50%.  Move to left side of tag.  Duplicate and mirror.
(see tag for placement)
Element 38 - C/P and re-size 50%.  Move to left side of tag.  Duplicate and mirror.
(see tag for placement)
Element 18 - C/P and re-size 30%.  Move to bottom middle of tag over middle skull.

Element 8 - C/P and re-size 20%.  Move to far left side and move over left most skull.
(see tag for placement)
Element 83 - C/P and re-size 30%.  Move to far left over right most skull.
(see tag for placement)
Element 63 - C/P and re-size 50%.  Move to bottom middle of left side.
(see tag for placement)
Element 28 - C/P and re-size 50%.  Move to very bottom middle of tag.
(see tag for placement)
Element 16 - C/P and re-size 35%.  Move to right side of element 28.
(see tag for placement)
Element 17 - C/P and re-size 30%.  Move to left side of element 16.
(see tag for placement)           

Add drop shadow to layers of your choosing.
I used
V & H 2
Opacity 70
Blur 7
Color Black

 Now you can add name and  © information.  Save as .png and re-size if you wish.
I re-size before I save and add name and © info.
