This tutorial was written by me on March 7, 2015.
It was made out of my own creation and any similarity is purely coincidental.
I used PSP X6 but any version would work.

Mask: WSL 3 & 84  HERE

Let’s start with the mask layer: 
New Raster – 800 X 800.
Open Paper 3.  Open mask (WSL 3) and minimize.
SELECTIONS – SELECT ALL – Copy and paste Paper 3 into selection.
Go to LAYERS – NEW MASK LAYER – FROM IMAGE – SOURCE LUMINANCE - CHECKED – from menu, find WSL 3 – OK.  
Duplicate and merge down.
NEW RASTER LAYER - Open next mask (WSL 84) and minimize.
SELECTIONS - SELECT ALL - Copy and paste Paper 4 into selection.
Go to LAYERS - NEW MASK LAYER - FROM IMAGE - SOURCE LUMINANCE CHECKED - from menu, find WSL 84 - OK.
Duplicate, merge down then re-size 115%.

This will be the frame layer:   
Frame 1 – C/P and re-size 95%.  Leave it where it is.   
Element 4 - C/P over Frame 1 and re-size 75%.  Make sure element 4 layer is still highlighted.  Grab your magic wand.  Click in the middle of frame.  SELECTIONS - MODIFY - EXPAND 8 pixels.  NEW RASTER LAYER - C/Paste into selection Paper 6.  Keep selected.  Now grab the tube you are using.  If you are using the same one as me, grab the Close Up now.  C/P and move her into position.
SELECTIONS - INVERT - DELETE - SELECTIONS - INVERT.
Element  10 - C/P and move to left side of frame over paper layer.  Copy twice and re-size 1 copy 75%.  Mirror and move to lower left part of frame but over keep over paper layer.  Re-size other copy of element 10 by 50%.  Move to middle left side of frame, but over paper layer.

Now onto some elements: 
Element 5 - C/P and re-size 75%.  Move to right corner of tag.  Make sure this layer is UNDER the frame layer(s).  Duplicate and mirror.

Element 9 - C/P and move to top middle of tag.  Make sure this layer is also UNDER the frame layer(s).  Duplicate and move to lower middle of tag.

Element 12 - C/P and re-size 50%.  Move to top upper left side of frame 1.  Make sure this layer, and all other layer(s), are OVER the frame layer(s).

Element 20 - C/P and move to upper right side of frame 1.

Element 16 - C/P and move to bottom left side of element 4.  Duplicate and mirror.

Word Art 1 - C/P and re-size 75%.  Move to top middle of tag.

I placed the tube layer(s) here: 
If your gonna use the same tube as I did, C/P the full version of the tube.  Mirror and move her to the bottom left side of tag.

Add drop shadow to layers of your choosing.
I've used
V & H 3
Opacity 70
Blur 7
Color Black

 Now you can add name and  © information.  Save as .png and re-size if you wish.
I re-size before I add name and © info and then save.
