This tutorial was written by me on January 5, 2014.
It was made out of my own creation and any similarity is purely coincidental.
I used PSPx2 but any version will do.

Here goes:
 
The Template first:
Open template in PSP.  Hold down SHIFT button and tap your “D” key.  Close the original template.  Delete the “Credits” layer with her CR info.

Next I changed the canvas size to 800 X 800.

We are gonna do basically the same thing for the following layers, so in an effort to stop repeating myself, here is what you will do for each layer that we change out:
Highlight layer.  SELECTIONS - SELECT ALL - FLOAT - DE FLOAT
Open Paper(s) - C/P - SELECTIONS - INVERT - DELETE - SELECTIONS - SELECT NONE
Delete layer. 
  
So it will go something like this:
You will be changing these layers: Rectangle, Square(s) 1,2,3, XeracX, and Certified.  We are not changing Certified Background or Stalker.
 
Highlight Rectangle.  Using directions above.  Use Paper 5.  Rename it Paper 5. 
Highlight Square 1, 2, 3.  Same as above.  Use Paper 8.  Rename Paper 8. 
Highlight XeracX.  Same as above.  Use Paper 10.  Rename Paper 10.  Move this layer to top left corner of tag.
Highlight Certified.  Same as above.  Use Paper 3.  Rename Paper 3.  Move this layer to top left corner of tag.

We are gonna use our tube now:
Open tube, choose your favorite version.  
Highlight the Paper 10 layer and go to SELECTIONS - SELECT ALL - FLOAT - DE-FLOAT - INVERT.
C/P tube layer, move into position and then hit DELETE.  Duplicate tube layer.  On the copy of tube layer:
ADJUST – BLUR – GAUSSIAN BLUR – RADIUS 25 - OK
Change BLEND MODE of copy of tube layer to HARD LIGHT and go to original tube layer and change BLEND MODE to LUMINANCE (L).

Now for more elements: 
Rose Heart - C/P and move this layer over Paper 5.  Move it to the middle of this layer.

Bow & Arrow - C/P and make sure this layer is over all other layers.  Move to left middle of tag.  Duplicate and mirror.

Scattered Hearts - C/P and leave where it lands.
(see tag for placement)
Lipstick Kiss - C/P and re-size 40%.  Move to top right corner of tag.

Again with the tube:
If you are using the same tube as me, C/P the version your choosing to use.  Re-size by 60%. 

Some More Elements:
Flowers - C/P and move to bottom middle of tag over tube.

Gift Heart - C/P and re-size 40%.  Move to bottom left corner of tag.  Duplicate and mirror.

Now for the mask layer:
Open Paper 4 and C/P.  Open WSL 84, then minimize. 
Highlight very bottom layer.  SELECTIONS - SELECT ALL
Copy and paste Paper 4 into Selection.
Go to LAYERS – NEW MASK LAYER – FROM IMAGE – SOURCE LUMINANCE CHECKED – CHOOSE WSL 84 from the drop down menu – OK.  Duplicate and merge down.  Re-size 115%.

Add drop shadow to layers of your choosing.
I used
V & H 2
Opacity 70
Blur 7
Color Black

 Now you can add name and  © information.  Save as .png and re-size if you wish.
I re-size before I save and add name and © info.
