This tutorial was written by me on January 20, 2015.
It was made out of my own creation and any similarity is purely coincidental.
I used PSP X6 but any version would work.

Mask: MPM 44  HERE

Let’s start with the mask layer: 
New Raster – 800 X 800.
Open Paper 10.  Open mask (MPM 44) and minimize.
SELECTIONS – SELECT ALL – Copy and paste Paper 10 into selection.
Go to LAYERS – NEW MASK LAYER – FROM IMAGE – SOURCE LUMINANCE - CHECKED – from menu, find MPM 44 – OK.  
Duplicate and merge down.

This will be the frame layer:   
Frame 5C/P and re-size 50%.  Free rotate 20º left.  Move up and just a little bit left on the tag.  Make sure element 42 layer is still highlightedGrab your magic wand.  Click in the middle of frame.  SELECTIONS - MODIFY - EXPAND 10 pixels.  NEW RASTER LAYER - C/Paste into selection Paper 1.

Now onto some elements: 
Element 20 - C/P and move to upper left side of tag.  Make sure this layer is UNDER the frame layer(s).

Element 40 - C/P and move to upper left corner of tag.  Make sure this layer is UNDER the frame layer(s).

Element 5 - C/P and move to upper right side of tag.  Make sure this layer is UNDER the frame layer(s).

Element 30 - C/P and re-size 50%.  Place over paper and frame layers but over to the right side of paper layer.  Make sure this layer, and all other layer(s), are OVER the frame layer(s).

Element 21 - C/P and re-size 50%. Place over paper layer on the left side of element 30.

 Element 29 - C/P and re-size 50%.  Place over paper layer on left side of element 21.

Element 11 - C/P and re-size 50%.  Place over paper layer on right side of element 21 and in front of element 21.

Element 12 - C/P and re-size 50%.  Place over paper layer on left side of element 29.

Element 33 - C/P and re-size 50%.  Place over paper layer in front of layers 30, 21, 29, 11 and 12.

Element 36 - C/P and re-size 50%.  Place over paper layer over element 12.

Tube Layer - I now put the tube here on the bottom right side of tag in front of element 5.  I C/P the tube, re-size 20% and move into position.

Element 25 - C/P and move to bottom middle of tag.  Make sure it covers the bottom of tube.

Element 32 - C/P and re-size 75%.  Move to bottom left corner of tag.  Grab your freehand tool and go along the inside of mirror.  Duplicate the tube and mirror.  Move over element 32 and go to SELECTIONS - INVERT - DELETE - SELECTIONS - SELECT NONE.  Change the opacity of the tube to 30%.

Element 28 - C/P and move to bottom left side of tag in front of element 32.  Duplicate and move a bit more left.

Add drop shadow to layers of your choosing.
I've used
V & H 2
Opacity 70
Blur 7
Color Black

 Now you can add name and  © information.  Save as .png and re-size if you wish.
I re-size before I add name and © info and then save.
